<p>Description</p> <p>To perform this position successfully, an individual must be able to perform each essential function of the position satisfactorily. Tippecanoe County provides reasonable accommodations to qualified employees and applicants with known disabilities who require accommodation to complete the application process or perform essential functions of the job, unless those accommodations would present an undue hardship.</p> <p>Incumbent serves as the Grant Funded Fetal & Infant Mortality Review and Prevention Nurse in the Tippecanoe County Health Department Public Health Nursing Division responsible for developing, coordinating, and evaluating projects related to West Central Indiana FIMR, and serving as liaison with healthcare providers, social service organizations, and civic and community agencies.</p> <p>DUTIES:</p> <ul> <li>Establishes and maintains integrated and active FIMR processes including, but not limited to, analyzing and monitoring community health indicators affecting fetal and infant mortality, performing maternal interviews, investing/coordinating and performing research, and preparing correspondence. </li><li>Provides immunizations to infants and parents and continued support of nursing division with immunizations and follow up as needed. </li><li>Coordinates with community partners and performs community outreach within twelve county region. </li><li>Maintains policies, plans, protocols, and guidelines reviewing, evaluating, and making recommendations, and assisting in implementing new legislation relating to fetal and infant mortality. </li><li>Provides orientation/training and education and research support to related hospitals, community service organizations, and health department personnel developing consumer and professional materials as needed. </li><li>Facilitates effective coordination between internal and external FIMR participants, developing and distributing reports relating to FIMR to hospital participants, Case Review Team (CRT) members, Department staff, Community Action Team (CAT) members, and local community service sources of referrals. </li><li>Assists in identifying capacity and capability of local health services and resources, searching for and applying to possible funding sources. </li><li>Attends meetings and serves on committees and taskforces as required. </li><li>Performs related duties as assigned. </li></ul> <p>Requirements</p> <p>I. JOB REQUIREMENTS:</p> <p>Associate's degree in nursing and minimum four years profession experience with current Indiana Registered Nurse license required. Bachelor's degree in nursing preferred.</p> <p>Working knowledge of and ability to make practical application of state and local public health regulations, Department policies and procedures, and accepted principles and practices of public health nursing.</p> <p>Working knowledge of standard immunization practices, procedures and potential complications, and issues in Maternal Child Health.</p> <p>Thorough knowledge of Fetal Infant Mortality Review process, data, and statistics related to fetal and infant mortality with demonstrated knowledge and experience in research.</p> <p>Ability to effectively examine and screen individuals, assess needs, and provide appropriate information, treatment, assistance and appropriate referrals. Ability to properly use various medical equipment, including stethoscope and syringes.</p> <p>Ability to effectively communicate orally and in writing with co-workers, local hospital personnel and health care providers, Case Review Team (CRT) members, Community Action Team (CAT) members, Health Information Management Staff, Indiana State Department of Health, State FIMR Coordinator, State of Indiana FIMR Program personnel, community organizations, Coroner's offices, and the public, including being sensitive to professional ethics, gender, cultural diversities, and disabilities.</p> <p>Working knowledge of standard English grammar, spelling and punctuation, and ability to properly use standard office equipment, including computer, printer, telephone and typewriter.</p> <p>Ability to understand and follow written and oral instructions, and work alone and with others in a team environment with minimum supervision.</p> <p>Ability to maintain accurate records, prepare detailed reports, and maintain confidentiality of Department records as required.</p> <p>Ability to work on several tasks at the same time and complete assignments effectively amidst frequent distractions and interruptions.</p> <p>Ability to serve on 24-hour call and respond swiftly, rationally and decisively to emergency situations.</p> <p>Ability to occasionally work extended, evening and/or weekend hours, and occasionally travel out of town for meetings/training, sometimes overnight.</p> <p>Possession of a valid driver's license and demonstrated safe driving record.</p> <p>II. DIFFICULTY OF WORK:</p> <p>Incumbent exercises discretion and independent judgment in modifying rules, instructions and guidelines to properly address specific cases and circumstances. Incumbent performs a broad array of duties which are not clearly related and involve analytical effort to diagnose.</p> <p>III. RESPONSIBILITY:</p> <p>Incumbent uses standard practices, policies and procedures to apply to specific situations and tasks. Incumbent is responsible for addressing unusual problems and/or circumstances and may discuss with supervisor. Work product is assessed for attainment of desired results, soundness of judgment, and general conformity with developed standards and policies.</p> <p>IV. PERSONAL WORK RELATIONSHIPS:</p> <p>Incumbent maintains frequent contact with co-workers, local hospital personnel and health care providers, Case Review Team (CRT) members, Community Action Team (CAT) members, Health Information Management Staff, Indiana State Department of Health, State FIMR Coordinator, State of Indiana FIMR Program personnel, community organizations, Coroner's offices, and the public, for the purpose of exchanging and explaining information, rendering service, and teaching/presenting information.</p> <p>Incumbent reports directly to Nursing Supervisor.</p> <p>V. PHYSICAL EFFORT AND WORK ENVIRONMENT:</p> <p>Incumbent performs majority of duties in a standard office environment and at various residential sites, such as patient homes and institutions. Duties involve standing/walking for long periods, pushing/pulling/lifting/carrying supplies/equipment weighing under 50 pounds, reaching, bending at waist, crouching/kneeling, close/far vision, color and depth perception, hearing sounds/ communication, and handling/grasping/fingering objects. Incumbent is exposed to normal hazards associated with driving, such as adverse weather conditions, and normal hazards associated with public health nursing, such as communicable diseases and blood-borne pathogens, for which universal health precautions must be followed to ensure safety of self and others. Incumbent occasionally works extended and/or evening hours, and occasionally travels out of town for meetings/training, sometimes overnight.</p>
