<p>Summary</p> <ul> <li> <p>POSITION INFORMATION</p> </li><li> <p>Position summary:</p> </li><li> <p>The Population Health Coordinator supports SGMG's quality management goals related to improving the health of the SGMG patient population by using advanced skills in data analytics, reporting, and developing tools to assist in making data actionable. This position, under the supervision of SGMG Administration, will provide ongoing, support, facilitation and coordination between new and established physicians and new and existing staff for population health activities for SGMG by communicating with patients and their insurance's, scheduling patients, performing clerical and clinical applications, and submitting data for various quality measure incentive programs. The incumbent will participate in quality improvement projects and work in collaboration with clinical and operational team members to support the quality teams in achieving goals and objectives.</p> </li><li> <p>MINIMUM QUALIFICATIONS</p> </li><li> <p>Education:</p> </li><li> <p>High school graduate, minimum; certification from approved school with certificate in Medical assisting preferred. Medical terminology required. Able to perform computer entry and basic office computer skills.</p> </li><li> <p>Required length and type of experience:</p> </li><li> <p>Externship of at least 100 hours or 3 years Medical Assistant experience in an outpatient setting.</p> </li><li> <p>Required licensure, certification or registry:</p> </li><li> <p>Medical Assisting certificate preferred.</p> </li></ul> <p>Additional Information</p> <p>SP0</p>
