<p>Rydal Park, a premier Life Plan Community located in Jenkintown, is hiring a Dementia Care Navigator Per-diem to join our team!</p> <p>Under general supervision, and with the support of an interdisciplinary team, the care navigator will actively partner with program beneficiaries and caregivers to find solutions that empower beneficiaries and caregivers to live their best lives. The Care Navigator develops awareness of beneficiaries' interests and needs and assists beneficiaries to maximize their independence and participation. The Care Navigator will be trained to provide coordination of care to beneficiaries ranging from basic needs to activities of daily living, to recreational activities. In collaboration with other team members, the Care Navigator will support a safe environment in which care is provided.</p> <p>To be successful in the role, you would have:</p> <ul> <li>Prior experience working with People Living with Dementia </li><li>Desire to work in an environment where you interact with & provide service to our Residents daily </li><li>Ability to complete initial 20-hour training and demonstrate comprehension of key principles. </li><li>Ability to complete training for Certified Dementia Practitioner after an introductory period. </li></ul> <p>What's in it for you?</p> <p>As one of the largest nonprofit owner/operator of senior living communities in the country, we are more than just a place to work.
