Join us in making a difference!</p> <p>Position Overview</p> <p>As a Home Care Coordinator, you'll play a vital role in advancing Trinity Health at Home's (THAH) mission by facilitating seamless, patient-centered care transitions. You'll be responsible for efficiently coordinating and enhancing the referral process, supporting patients as they transition from acute care settings to post-acute home care.
