This position is responsible for assessing patient needs, implementing care plans, and providing education to patients and their families. The LPN will work closely with a multidisciplinary team to ensure that all aspects of patient care are addressed effectively. By maintaining accurate patient records and monitoring vital signs, the LPN contributes to the overall health and well-being of residents. Ultimately, this role is vital in fostering a safe and nurturing atmosphere for all patients, enhancing their quality of life.</p> <ul> <li>Administer medications and treatments as prescribed by the healthcare provider. </li><li>Conduct patient assessments and document findings in electronic health records. </li><li>Provide wound care and assist with daily living activities to ensure patient comfort. </li><li>Educate patients and their families on health management and care plans. </li><li>Collaborate with healthcare team members to develop and implement individualized care plans. </li></ul> <p>Our ideal candidate</p> <p>Minimum Qualifications</p> <ul> <li>Current and valid Nursing License as a Licensed Practical Nurse. </li><li>Proven experience in patient care, particularly in a long-term care setting. </li><li>Ability to read, write, and speak English fluently. </li></ul> <p>Preferred Qualifications</p> <ul> <li>Certification in wound care management. </li><li>Experience in patient education and health promotion. </li><li>Bilingual skills to enhance communication with diverse patient populations. </li></ul> <p>Skills</p> <p>The required skills of wound care, patient education, and nursing expertise are essential for the daily responsibilities of the LPN.
