HealthDrive connects patients in need of vital healthcare to doctors committed to dignity and excellence.</p> <p>Responsibilities</p> <p>The Licensed Mental Health Counselor (LMHC) will provide behavioral health services to the residents of nursing homes, assisted living facilities, and long-term care facilities within an assigned territory.</p> <p>Essential Functions:</p> <ul> <li>Complete psychosocial (initial) assessments </li><li>Provide 1-on-1 therapy to patients </li><li>Recommend behavioral/therapeutic interventions to nursing home staff </li><li>Conduct risk assessments as needed </li><li>Enter patient information and required clinical documentation in EMR system </li><li>Perform other duties as assigned </li><li>Mobile model treatment of patients requires daily travel to skilled nursing facilities </li></ul> <p>Qualifications</p> <ul> <li>Master's degree in mental health counseling or a related field from an accredited college or university </li><li>State professional LMHC license </li><li>Valid driver's license </li><li>Ability to work independently on a daily basis </li><li>Excellent written, verbal, interpersonal and organizational skills </li><li>Ability to use email, the internet, and to learn other healthcare related software </li></ul>
