And above all, it means exceptional care, simply delivered - and we couldn't do it</p> <p>without you.</p> <p>Make a Difference</p> <p>The Life Skills Clinician (LSC)/Life Skills Specialist (LSS) will engage with individuals and families to improve their health and wellness, live self-directive lives, and strive to reach their full potential. This role is responsible for conducting crisis response and de-escalation to individuals in the community and via the crisis response hotline. The team will partner with first responders and community partners to reduce arrests and need for support from emergency departments. Ongoing support to individuals and families will be given through targeted outreach, care coordination, and</p> <p>engagement support.</p> <p>Exceptional Skills and Qualifications</p> <ul> <li>Patients First: The Mobile Crisis Life Skills Clinician/Life Skills Specialist collaborates as part of a multidisciplinary care team by supporting the comprehensive psycho-social assessments. They help clients regulate emotions and document services provided in the Electronic Medical Record (EMR) based on policies and procedures. When necessary, the Life Skills Clinician/Life Skills Specialist will transport clients in a Network vehicle. They meet client's where they are at by providing crisis response in client's homes, in community settings, at crisis departments, or at emergency departments. </li><li>Relationships: Applicants for this position should be able to collaborate with others in a team setting, have excellent communication skills, and a positive attitude toward problem-solving. They will assist with crisis and emergency departments when not responding to off-site crisis calls. </li><li>Integrity: The Mobile Crisis Life Skills Clinician/Life Skills Specialist will comply with all applicable Behavioral Health Care Service policies and procedures. </li><li>Inclusion: Candidates must have the ability to work with clients in various stages of change who need assistance regulating thoughts and emotions. </li><li>Diversity: The ideal candidate will be able to acknowledge, value, and celebrate collective experiences and differences of patients, other caregivers, and the communities we serve. </li><li>Excellence: The Mobile Crisis Life Skills Clinician/Life Skills Specialist will be actively involved in professional development and education, including mandatory training.
