Learn more about our work at healthsolutions.org.</p> <p>PHS administers WholeYouNYC (WYNYC), a coordinated community resource network that builds trustworthy and reliable pathways between healthcare providers, health plans and CBOs providing critical resources in the community that address the social drivers of health. WYNYC brings together over 100 organizations offering various programs - such as food, housing, employment, health insurance, and sexual health services - across all five boroughs. These services and programs make it possible for New Yorkers to live their healthiest lives and ultimately reduce health disparities and advance health equity. To date, our network has already impacted thousands of lives through community partnerships and referrals, generating millions in estimated healthcare savings.</p> <p>New York State (NYS) recently announced the availability of $500M statewide to support Social Care Network (SCN) lead entities responsible for coordinating social care delivery in various regions across the state. Public Health Solutions (PHS) and our WYNYC network were awarded the role of regional SCN for Brooklyn, Manhattan, and Queens.</p> <p>This is a grant-funded position ending March 31, 2027.</p> <p>Position Summary:</p> <p>We seek an experienced manager to oversee WholeYouNYC-SCN's training and technical assistance activities for health-related social needs (HRSN) service providers ("network partners"). The manager will work closely with the Healthcare-Community Partnerships Capacity Building Team, other network care delivery teams and capacity building partners to enhance HRSN service provider SCN knowledge capacity and engagement. The Manager, Training and Technical Assistance will draw upon SCN care delivery and social services navigation expertise to ensure training and technical assistance activities maximize value for network partners.</p> <p>Specifically, the Manager, Training and Technical Assistance will:</p> <ul> <li>Manage partner training and technical assistance activities for the SCN promoting a culture of engagement, network quality and efficiency. </li><li>Develop a mastery level understanding of the SCN care delivery process and act as a subject matter advisor providing leadership, guidance and best practice techniques to network staff and partners. </li><li>Collaborate with other teams and network partners to develop and implement a comprehensive SCN training curricula for network partners. </li><li>Collaborate with other teams and network partners to oversee designation of the SCN's screening and navigation workforce ensuring their readiness to provide services on behalf of the network. </li><li>Contribute to the design and implementation of a comprehensive partner onboarding program supporting documentation and tool development, as needed. </li><li>Manage the SCN partner onboarding process, working collaboratively with other internal teams and external partners, as needed, to enable seamless and efficient onboarding onto the network. </li><li>Develop and oversee the management and maintenance of an SCN Knowledge Hub ensuring a repository of readily available and accessible resources for network partners. </li><li>Design and implement tailored technical assistance solutions and processes that enhance CBO knowledge capacity and enable their successful participation in the SCN. </li><li>Plan and oversee network technical assistance activities to include, but not limited to partner 1:1s, office hours, CBO roundtables and other engagement events and forums. </li><li>Design, develop and implement monitoring and evaluation methods to assess the effectiveness of network training and technical assistance initiatives. </li><li>Lead and/or support network partner trainings including direct support for and management of materials / plan / schedule development, delivery and post training evaluation and analysis. </li><li>Document best practices and lessons learned from training and technical assistance activities and propose and implement SCN continuous improvement recommendations. </li><li>Work collaboratively with other SCN care delivery teams and Quality Improvement & Evaluation to develop and implement performance monitoring protocols for SCN care delivery activities. </li><li>Oversee the development and execution of a CBO performance evaluation plan and scorecard. </li><li>Develop and implement corrective action plans to address identified CBO performance issues. </li><li>Provide direct oversight, supervision and performance management for Training and Technical Assistance Coordinators supporting the WholeYouNYC-SCN. </li><li>Perform other related duties, as assigned. </li></ul> <p>Qualifications and Experience:</p> <ul> <li>Bachelor's Degree preferably in public health, health education or a related field and/or equivalent. </li><li>Minimum 3 years of experience in a project management, managerial or supervisory role and/or equivalent. </li><li>Proven professional experience developing and delivering training and/or technical assistance, preferably in a public health setting. </li><li>Strong professional knowledge of effective social needs screening and referral practices. </li><li>Understanding of the health and human services sector in NYC/NYS and related government agencies. </li><li>Foundational knowledge of the social drivers of health (SDoH) and their impact on health outcomes. </li><li>Excellent interpersonal skills (verbal, written and presentation). </li><li>Strong organizational skills with the ability to handle multiple tasks simultaneously. </li><li>Excellent attention to detail, accuracy and dependability. </li><li>Ability to travel occasionally across Brooklyn, Queens and Manhattan. </li></ul> <p>Desired Skills:</p> <ul> <li>MPH, MPA or related Master's Degree. </li><li>Bilingual language skills. </li><li>Experience working with Medicaid. </li></ul> <p>Benefits:</p> <ul> <li>Hybrid Work Schedule. </li><li>Generous Paid Time Off and Holidays. </li><li>An attractive and comprehensive benefits package including Medical, Dental and Vision. </li><li>Flexible Spending Accounts and Commuter Benefits. </li><li>Company Paid Life Insurance and Disability Coverage. </li><li>403(b) + employer matching and discretionary company contributions. </li><li>College Savings Plan. </li><li>Ongoing trainings and continuous opportunities for professional growth and development. </li></ul> <p>At PHS, we place immense value on diversity within our teams, understanding that varied backgrounds and experiences significantly enhance our community and propel us toward our goals.
