<p>Job Summary</p> <p>The Outreach Coordinator is responsible for coordinating wrap around services for pregnant and postpartum women with Opioid Use Disorder (OUD). In conjunction with partner agencies, the goal is to optimize the health of this diverse population of women, and their ability to provide care for their Substance Exposed Newborns (SEN). Additionally, the Outreach Coordinator is responsible for implementation and management of the comprehensive program of care, referrals and education. The Outreach Coordinator works closely with BMC OB providers, Family Birthplace staff and leadership, pediatric providers and staff, Medication Assisted Treatment (MAT) providers and programs, and partner agencies across the continuum, to deliver these services. This position requires participation in OB, pediatric and community partner telehealth or in-person visits to continually strengthen the relationship with the family.</p> <p>Experience</p> <ul> <li>Two years of experience in a clinical setting. Prior experience working with individuals to assess their needs and assist in goal setting is preferred. </li><li>Lived experience chronic pain or substance use disorder is preferred. </li></ul> <p>Education</p> <ul> <li>High School diploma is required, and an Associate's degree, nursing education or Community Health Worker designation is preferred. </li><li>Must complete all mandatory trainings required by the department. </li></ul> <p>Additional Requirements</p> <ul> <li>Familiarity of harm reduction, strength based motivational interviewing, trauma informed care </li><li>Comfort administering pain, depression, social determinants of health and substance use screening tools </li><li>Proven ability to effectively collaborate with individuals/agencies, optimize outreach activities, and participate in program planning & implementation. </li><li>Experience with tracking, trending & analyzing data to create the action plans for performance improvement. </li><li>Working knowledge of community and health system resources </li><li>Excellent interpersonal skills with a variety of people including patients, families, staff, physicians, and agencies required. </li><li>Proven ability to exercise independent judgment and initiative. </li></ul> <p>Schedule:</p> <p>Monday -Friday</p> <p>8:00am -4:30pm with some rate weekends and evenings commitments</p>
