<p>Summary of Position: Provides clinical leadership in the oversight of nursing care within the physician practice setting.# Responsible for coordinating the clinical and administrative functions that govern the provision of care within the practice.# Works closely with the providers and the practice manager to ensure an environment that is caring, healing and welcoming for patients. Primary Job Responsibilities: Policies and Procedures: Responsible for implementing and maintaining nursing policies and procedures for the practice.# Participates in quality improvement activities, developing policies, procedures, and/or guidelines to improve patient care and assists with data collection and monitoring. Scheduling: Schedules registered nurses and other clinical staff to provide optimal coverage for patient care.# Organizes and prioritizes work assignments based on patient needs and acuity, age and utilizes critical thinking skills. Responds to emergency situations as per protocol. Patient Assessment: During patient visit, assesses patient#s physical/psycho-social status, reviews medications and confirms medication reconciliation as appropriate.# Obtains vital signs, height and weight.# Patient Care: Collaborates with the provider to formulate and implement a plan of care as appropriate based on assessment and diagnosis.# Refers patient to case manager/social worker to ensure access to additional services and resources.# Assures consistency in standards of care and the provision of care meets all applicable joint commission standards to ensure patient safety.# Triages calls from patients, family members and community health providers.# Patient Education: Provides information to patient/family with consideration to individual needs.# Assesses for barriers to learning and motivation.# Human Resources: Provides ongoing feedback to staff to nurture and develop clinical and interpersonal skills.# Proactively addresses issues of conflict.# Conducts annual performance evaluations.# Ensures staff complete mandatory training as required.# Takes corrective action as needed. Communication:#Applies patient specific communication skills in interactions that are culturally sensitive and population/age specific.# Manages conflict among other team members in a direct and effective manner. Leadership: Applies leadership skills consistent with role and experience.# Delegates and/or assigns care that is consistent with role, scope of practice, and abilities of recipient.# Coordinates care within the multi-disciplinary team.# Actively participates and promotes professional practice by participating on committees, councils, policy development, unit/hospital projects or professional organizations. Minimum Qualifications: Graduate of an accredited school of nursing, BSN required.# Minimum two years nursing experience required, primary care experience required.# Minimum of three years supervisory experience preferred.
