<p>Licensed Social Worker-Dialysis</p> <p>Full-Time</p> <p>Queens-Long Island Renal Institute, located on the lobby level of the renowned Parker Jewish Institute for Healthcare and Rehabilitation in New Hyde Park, New York, is seeking a Full-Time Social Worker, with a strong background in Renal Social Work, to join our team of professionals.</p> <p>QLIRI provides state-of-the-art in-center hemodialysis treatment for kidney disease patients. Our experienced interdisciplinary team of nephrologists and renal nurses offer chronic dialysis services in a comfortable and modern setting. Located on the Queens-Nassau boarder, Queens-Long Island Renal Institute has a longstanding reputation for the finest quality of care, state-of-the-art technology and uncompromised dignity.</p> <p>The Social Worker provides services in the dialysis unit to patients and their families to support and maximize the social functioning and adjustment of the patient with End Stage Renal Disease.</p> <p>Key Tasks Include:</p> <ul> <li>Conducts initial patient psychosocial assessment and comprehensive plan of care </li><li>Provides support, information and assistance in treatment planning </li><li>Develops and continues to monitor psychosocial goals for plan of care </li><li>Counsels new patients and families on pre-dialysis education, including treatment options </li><li>Acts as advocate on behalf of patient and family </li><li>Completes and documents initial and ongoing comprehensive assessment, evaluations and quarterly notes, along with documenting and monitoring ongoing psychosocial issues and patient goals </li><li>Facilitates and/or refers patients and families to support/education groups </li><li>Facilitates education on Advance Directives and treatment modalities </li><li>Coordinates and participates in patient/family care conferences with interdisciplinary team </li><li>Analyzes and provides information and facilitates referrals to community, private, state and federal agencies based on psychosocial needs </li><li>Provides crisis intervention expertise as necessary </li><li>Provides assistance to patient and family in adjusting to dialysis and altered lifestyle, and provide resource information </li><li>Arranges patient travel arrangements for both guest and permanent patients and ensure completion of arrangements </li><li>Coordinates patient transfers/ visit to other facilities </li><li>Provides assistance to patient regarding health insurance </li><li>Maintains patient confidentiality and administers patient care in a respectful manner </li></ul> <p>Position Requirements:</p> <ul> <li>Licensed Social Worker in New York State; LMSW required; LCSW a plus </li><li>At least two (2) years of prior work experience as a Social Worker </li><li>Prior experience as a Dialysis Social Worker or working with dialysis patients </li><li>Strong interviewing skills for patient evaluation, problem solving, and recording </li><li>Possess strong analytical skills to assist patients in problem solving and resolution </li><li>Capable of communicating clearly and effectively with all types of physically and mentally challenged patients and their families </li><li>Professional, flexible and a team player </li></ul> <p>This position offers a competitive rate of pay supported by an excellent benefit package including health insurance, 401(k), paid time off, free parking and more.</p> <p>Qualified applicants are encouraged to apply immediately!</p>
