<p>POSITION SUMMARY:</p> <p>This employee provides individual psychotherapy, specialized group counseling, individual and group</p> <p>family therapy, family outreach and care management services to adults with Substance Use</p> <p>Disorders (SUD) and/or co-occurring Mental Health Disorders and SUD, so they may achieve stable</p> <p>mental health and sustained remission from SUD, restore and increase healthy social connections,</p> <p>obtain stable income and permanent housing, and reduce their risk of harm, hospitalization,</p> <p>incarceration, and homelessness . The employee also conducts diagnostic assessments and provides</p> <p>consultation to Case Counselors serving clients with co-occurring conditions.</p> <p>ESSENTIAL FUNCTIONS:</p> <p>Provide individual psychotherapy, specialized group counseling and mental health care</p> <p>management to adults with co-occurring Mental Health and Substance Use Disorders.</p> <p>Facilitate specialized trauma informed group counseling and psychoeducation</p> <p>programming.</p> <p>Provide individual family needs assessments, individual family therapy, and group family</p> <p>education.</p> <p>Facilitate specialized group education on the role of families in SUD recovery. Record all service activities by entering progress notes in the Y-Haven Electronic Medical Record.</p> <p>Establish and maintain regular collaborative relationships with social service organizations and programs that residents may utilize for mental healthcare and vocational rehabilitation.</p> <p>Participate in team meetings, in-service trainings and coordination meetings.</p> <p>Coordinate with Clinical Director on all critical clinical issues.</p> <p>Conduct Diagnostic Assessments.</p> <p>Provide mentoring and support to Case Counselors regarding mental health, family, and intimate relationship matters.</p> <p>Other duties as assigned.</p> <p>QUALIFICATIONS:</p> <p>Education:</p> <p>Required Education: Master's Degree in relevant field.</p> <p>Experience:</p> <p>Must have at least 5 years' experience in behavioral health counseling and psychotherapy</p> <p>and/or Family Therapy.</p> <p>Technical Skills and Knowledge:</p> <p>Proven ability to meet minimum productivity requirements (five hours per day of direct reimbursable clinical care).</p> <p>Must have successful experience facilitating behavioral health group counseling.</p> <p>Preferred License: LISW (Licensed Independent Social Worker) or LPCC (Licensed</p> <p>Professional Clinical Counselor).</p> <p>The Y: We're for youth development, healthy living, and social responsibility.</p> <p>Required License: LSW (Licensed Social Worker) or LPC (Licensed Professional Counselor).</p> <p>Must have extensive knowledge of Substance Use Disorders and other issues affecting the homeless population.</p> <p>Must have basic computer skills, using Microsoft Office applications.</p> <p>Should have working knowledge of OHMAS, CARF and VA program compliance standards.</p> <p>Bilingual (Spanish/English) preferred, but not required</p>
