This role is responsible for timely and effective clinical care, collaboration with a multidisciplinary team, spiritual outreach, and participation in organizational initiatives that promote wellness, efficiency, and patient empowerment.</p> <p>Key Responsibilities</p> <p>Clinical Excellence & Team-Based Care</p> <ul> <li> <p>Arrive and start sessions on time; prepared for AM/PM patient sessions</p> </li><li> <p>Participate in and lead daily team huddles</p> </li><li> <p>Delegate effectively to support staff and maintain patient flow</p> </li><li> <p>Follow up on hospital discharges, test results, referrals, and medication refills</p> </li><li> <p>Provide efficient, high-quality patient care, completing documentation within 24 hours</p> </li><li> <p>Engage in cost-effective, evidence-based care aligned with EHC clinical guidelines</p> </li><li> <p>Prescribe responsibly, especially regarding controlled medications</p> </li><li> <p>Handle patient needs efficiently during on-call periods and complete follow-up documentation</p> </li></ul> <p>Patient Engagement & Empowerment</p> <ul> <li> <p>Educate and empower patients to take an active role in their own wellness</p> </li><li> <p>Schedule follow-ups strategically for chronic condition management</p> </li><li> <p>Set clear visit agendas and boundaries with patients</p> </li><li> <p>Provide culturally sensitive and spiritually integrated care</p> </li><li> <p>Encourage patient involvement in decision-making and follow-up compliance</p> </li></ul> <p>Documentation & Follow-Up</p> <ul> <li> <p>Complete charting, referrals, and documentation on the day of the visit whenever possible</p> </li><li> <p>Review labs and communications bins regularly</p> </li><li> <p>Address refill and message requests within 2 working days</p> </li><li> <p>Complete patient forms within 5 working days</p> </li></ul> <p>Spiritual & Holistic Care</p> <ul> <li> <p>Provide spiritual care through prayer, scripture, and connection to churches when appropriate</p> </li><li> <p>Refer patients to behavioral health or social services as needed</p> </li><li> <p>Serve all patients with humility, love, and respect-regardless of spiritual interest or beliefs</p> </li><li> <p>Participate in daily staff devotions and contribute to the spiritual life of the team</p> </li></ul> <p>Collaboration & Culture</p> <ul> <li> <p>Resolve conflicts directly and respectfully with team members</p> </li><li> <p>Encourage team morale and model a positive, constructive attitude</p> </li><li> <p>Train, mentor, and precept students when appropriate</p> </li><li> <p>Suggest and support clinic-wide quality improvement efforts</p> </li><li> <p>Participate in fundraising and community outreach events (optional)</p> </li></ul> <p>Clinical Skills & Accountability</p> <ul> <li> <p>Demonstrate sound clinical judgment and stay current with care guidelines</p> </li><li> <p>Maintain up-to-date credentials (license, DEA, CME)</p> </li><li> <p>Seek supervision appropriately and adhere to licensing limitations</p> </li><li> <p>Follow organizational protocols, including on spiritual care boundaries and biblical ethics</p> </li></ul> <p>Qualifications</p> <ul> <li> <p>Licensed Nurse Practitioner (NP); Family Medicine preferred</p> </li><li> <p>At least 2 years experience in primary care</p> </li><li> <p>Demonstrated commitment to Christian values and ethical practice</p> </li><li> <p>Passion for serving underserved populations</p> </li><li> <p>Strong communication, teamwork, and organizational skills</p> </li><li> <p>Ability to integrate faith into clinical care in a respectful, patient-centered manner</p> </li><li> <p>Accountable to and active in a Bible-teaching church congregation</p> </li></ul> <p>COVID-19 Vaccine Requirement</p> <p>Esperanza Health Center requires all on-site workforce members, including employees, contractors, and volunteers, to be vaccinated against COVID-19.
