<p>Job Description</p> <p>Overview</p> <p>The Home Care LPN Scheduler at IU Health Home Health plays a vital role in ensuring the smooth delivery of home care services. This position involves coordinating with referral sources, MDs, and Home Care Case Managers to monitor IP referrals for discharge and ensure the start of care within the regulatory time frame. The LPN Scheduler confirms the provider to follow the patient during homecare services and is an integral part of the care delivery setup. This position requires LPN experience and proficiency in Excel.</p> <p>Key responsibilities include:</p> <ul> <li>Coordinating and scheduling home care services for clients based on their needs and preferences. </li><li>Communicating with home care staff to manage their schedules and assignments. </li><li>Ensuring that all shifts are covered and arranging for backup or replacement staff when necessary. </li><li>Maintaining detailed records of services provided, including dates, times, and specific tasks performed. </li><li>Managing and resolving scheduling conflicts or issues promptly and professionally. </li><li>Collaborating with healthcare professionals to ensure continuity of care for clients. </li><li>Assisting in the recruitment and training of new home care staff. </li><li>Performing administrative tasks such as data entry, file management, and report preparation. </li><li>Ensuring compliance with all relevant regulations and standards related to home care services </li></ul> <p>We offer a comprehensive benefits package designed for you, which includes Medical-Dental-Vision, a matching 401K, PTO, tuition reimbursement program, wellness program, and more!</p> <p>Schedule: Full Time | Day Shift | 40 Hours Per Week | Hybrid - with in-person meetings and occasional need to come to the office</p> <p>Hours: Monday - Friday | 8:00 a.m. - 4:30 p.m.</p> <p>Knows and uses the nursing process in planning, implementing, and evaluating nursing care to individual patients. Collaborates with other members of the healthcare team. Evaluates response to interventions and patient status toward goal achievement. Accepts delegated nursing measures for which he or she knows he or she is prepared, qualified and licensed to perform.</p> <ul> <li>Requires graduation from a nationally accredited licensed practical nursing program. </li><li>Requires an active Licensed Practical Nurse (LPN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) LPN license. </li><li>Requires Basic Life Support certification through the AHA. Other advanced life support certifications may be required per unit/department specialty according to patient care policies. </li><li>Successful completion of orientation required. </li><li>1-2 years of relevant experience preferred. </li></ul>
