<p>Are you looking for a great career? We have a great opportunity for you! CarDon and Associates is now hiring an experienced MDS Traveler (LPN)! We offer daily and weekly pay options and great benefits!</p> <p>The MDS Traveler will be assigned to 5 or 6 facilities to assist in timely completion of MDS, completion of level of cares, review accuracy of quality measures that trigger, and monitor accuracy of the Medicaid identifiers on face sheet.</p> <p>About the Role</p> <p>The MDS Traveler (LPN) at CarDon and Associates Corp plays a crucial role in ensuring the delivery of high-quality care to residents across various facilities. This position involves conducting comprehensive assessments and developing individualized care plans that meet the unique needs of each resident. The MDS Traveler will collaborate with interdisciplinary teams to ensure compliance with regulatory standards and enhance the overall quality of care. By traveling to different locations, this role provides flexibility and the opportunity to impact multiple communities positively. Ultimately, the MDS Traveler will contribute to the mission of CarDon and Associates Corp by promoting the well-being and dignity of every resident.</p> <p>Job Responsibilities</p> <ul> <li>Conduct MDS assessments and ensure accurate documentation of resident information. </li><li>Develop and implement individualized care plans in collaboration with healthcare teams. </li><li>Monitor and evaluate resident progress, adjusting care plans as necessary. </li><li>Provide training and support to facility staff on MDS processes and best practices. </li><li>Ensure compliance with state and federal regulations related to resident care. </li></ul> <p>Minimum Qualifications</p> <ul> <li>Current LPN license in the state of practice. </li><li>Experience in long-term care or skilled nursing facilities. </li><li>Knowledge of MDS assessment processes and regulations. </li></ul> <p>Preferred Qualifications</p> <ul> <li>Experience with electronic health record (EHR) systems. </li><li>Certification in MDS coordination or related fields. </li><li>Strong communication and interpersonal skills. </li></ul> <p>Skills</p> <p>The required skills for this role include a solid understanding of MDS assessments and the ability to navigate regulatory requirements effectively. Daily work will involve utilizing communication skills to collaborate with healthcare teams and provide training to staff. Preferred skills, such as proficiency in EHR systems, will enhance the efficiency of documentation and care planning processes. Strong analytical skills will be essential for evaluating resident progress and making necessary adjustments to care plans. Overall, both required and preferred skills will contribute to delivering exceptional care and improving the quality of life for residents.</p>
