We offer daily and weekly pay options and great benefits!</p> <p>The MDS Traveler will be assigned to 5 or 6 facilities to assist in timely completion of MDS, completion of level of cares, review accuracy of quality measures that trigger, and monitor accuracy of the Medicaid identifiers on face sheet.</p> <p>About the Role</p> <p>The MDS Traveler (LPN) at CarDon and Associates Corp plays a crucial role in ensuring the delivery of high-quality care to residents across various facilities. Ultimately, the MDS Traveler will contribute to the mission of CarDon and Associates Corp by promoting the well-being and dignity of every resident.</p> <p>Job Responsibilities</p> <ul> <li>Conduct MDS assessments and ensure accurate documentation of resident information. </li><li>Develop and implement individualized care plans in collaboration with healthcare teams. </li><li>Monitor and evaluate resident progress, adjusting care plans as necessary. </li><li>Provide training and support to facility staff on MDS processes and best practices. </li><li>Ensure compliance with state and federal regulations related to resident care. </li></ul> <p>Minimum Qualifications</p> <ul> <li>Current LPN license in the state of practice. </li><li>Experience in long-term care or skilled nursing facilities. </li><li>Knowledge of MDS assessment processes and regulations. </li></ul> <p>Preferred Qualifications</p> <ul> <li>Experience with electronic health record (EHR) systems. </li><li>Certification in MDS coordination or related fields. </li><li>Strong communication and interpersonal skills. </li></ul> <p>Skills</p> <p>The required skills for this role include a solid understanding of MDS assessments and the ability to navigate regulatory requirements effectively.
