<p>Social Worker/ Home Health</p> <p>Position Summary :</p> <p>Interim HealthCare is seeking a compassionate and dedicated Social Worker to join our team in providing quality home healthcare services to our patients. As a Social Worker with Interim HealthCare, you will play a crucial role in delivering personalized care to patients in the comfort of their homes. You will assess the social and emotional needs of patients, collaborate with an interdisciplinary team, and ensure that each patient has the resources and support needed to improve their health and quality of life. This role is currently for Per-Diem with potential to transition to part-time or full-time.</p> <p>Key Responsibilities :</p> <ul> <li> <p>Conduct comprehensive psychosocial assessments of patients in their homes to identify social, emotional, financial, and environmental factors affecting their well-being and care.</p> </li><li> <p>Develop and implement individualized care plans based on patient assessments, addressing psychosocial needs and promoting overall well-being.</p> </li><li> <p>Offer counseling and emotional support to patients and families, helping them cope with illness, injury, and caregiving challenges.</p> </li><li> <p>Assist patients and families in navigating complex healthcare systems, including accessing resources such as financial assistance, transportation, support groups, and community services.</p> </li><li> <p>Collaborate with physicians, nurses, and other healthcare professionals to ensure integrated, patient-centered care.</p> </li><li> <p>Facilitate communication and coordination among patients, family members, caregivers, and the healthcare team to optimize care delivery.</p> </li><li> <p>Provide discharge planning and support to patients transitioning from hospital care to home healthcare, ensuring continuity and proper follow-up care.</p> </li><li> <p>Educate patients and their families on disease management, coping strategies, and healthcare resources to improve quality of life.</p> </li><li> <p>Advocate for patients to ensure they receive the full spectrum of services, including home healthcare, equipment, medications, and therapy, when appropriate.</p> </li><li> <p>Maintain accurate documentation of patient assessments, interventions, and progress in compliance with agency policies, regulatory requirements, and HIPAA standards.</p> </li><li> <p>Attend regular team meetings and contribute to case discussions and patient care planning.</p> </li><li> <p>Stay up-to-date with industry best practices, regulations, and changes in the healthcare system.</p> </li></ul> <p>Qualifications :</p> <ul> <li> <p>Degree in Social Work from an accredited school of social work.</p> </li><li> <p>Current and valid state licensure (LCSW, LMSW, or equivalent) as a Social Worker.</p> </li><li> <p>Previous experience in home health care, hospital, or healthcare setting preferred.</p> </li><li> <p>Knowledge of social work principles, patient advocacy, and case management in the home health environment.</p> </li><li> <p>Strong interpersonal and communication skills, with the ability to engage patients, families, and healthcare professionals.</p> </li><li> <p>Ability to work independently, prioritize tasks, and manage a caseload.</p> </li><li> <p>Understanding of community resources, financial assistance programs, and healthcare regulations.</p> </li><li> <p>Valid driver's license and reliable transportation for patient visits.</p> </li></ul> <p>Physical Requirements :</p> <ul> <li> <p>Ability to travel to patient homes within the assigned geographic area.</p> </li><li> <p>Ability to lift and carry light equipment and supplies as necessary for patient assessments and care.</p> </li></ul> <p>Work Environment :</p> <ul> <li> <p>Primarily based in patients' homes within the community.</p> </li><li> <p>Flexible work hours, including availability for weekend and evening visits if needed.</p> </li><li> <p>Supportive, team-oriented work environment with opportunities for professional growth and development.</p> </li></ul> <p>Why Work for Interim HealthCare?</p> <p>Founded in 1966, Interim HealthCare is the nation's first home care company and a leading employer of Home Health Aides (HHAs).
