<p>HEALTHCARE OPPORTUNITY - CCM/RPM CARE COORDINATOR</p> <p>Healthy Connections, Inc.</p> <p>This position can be based at any of our current clinic locations or remotely depending on the applicant.</p> <p>Join our innovative team at Healthy Connections where we're expanding our Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) programs to better serve our communities across Arkansas.</p> <p>Position Overview:</p> <p>Seeking a dedicated healthcare professional to manage care for patients with chronic conditions through our state-of-the-art remote monitoring platform. You'll be part of a team making a real difference in patients' lives through proactive healthcare management.</p> <p>Preferred Experience:</p> <p>Candidates with direct experience in Medicare Chronic Care Management (CCM) and/or Remote Patient Monitoring (RPM) programs will be given priority consideration. Experience should include:</p> <ul> <li>Care plan development and management </li><li>Time-based care documentation </li><li>Remote monitoring platform usage </li><li>Medicare compliance requirements </li><li>G0511 billing familiarity </li></ul> <p>However, we will consider exceptional candidates with strong healthcare backgrounds who demonstrate the ability to quickly master our care management platform.</p> <p>Key Responsibilities:</p> <ul> <li>Monitor patient health data through our digital platform </li><li>Review and respond to patient alerts (critical alerts within 30 minutes) </li><li>Maintain regular patient communication </li><li>Document care management activities </li><li>Coordinate with healthcare providers </li><li>Ensure quality care metrics are met </li><li>Manage a caseload of up to 200 patients </li><li>Achieve required monthly time requirements </li><li>Maintain 90%+ patient satisfaction rate </li></ul> <p>Required Qualifications:</p> <ul> <li>Medical Assistant, LPN, RN, or similar healthcare background </li><li>2+ years healthcare experience </li><li>Strong computer skills </li><li>Experience with electronic health records, CGM/eMDs a plus </li><li>Excellent communication abilities </li><li>Detail-oriented with strong documentation skills </li></ul> <p>Preferred Qualifications:</p> <ul> <li>Direct CCM/RPM program experience </li><li>Knowledge of Medicare chronic care requirements </li><li>Experience with remote monitoring platforms </li><li>Understanding of G0511 billing requirements </li><li>Track record of successful patient engagement </li><li>Experience meeting quality metrics in similar role </li><li>Bilingual (English/Spanish) a plus </li></ul> <p>Quality Metrics You'll Maintain:</p> <ul> <li>Patient satisfaction: 90%+ </li><li>Response times: Critical alerts within 30 minutes </li><li>Documentation compliance: 100% </li><li>Monthly patient contact requirements </li><li>Care plan management </li><li>Regular provider communications </li></ul> <p>Experience with CCM/RPM programs is highly valued, but we will train the right candidate who demonstrates strong potential and commitment to excellence.</p> <p>Benefits:</p> <p>Healthy Connections offers a competitive employment package, including:</p> <ul> <li>Competitive salary </li><li>Health, dental, and life insurance for employees and families at discounted pre-tax rates </li><li>401(k) retirement plan with up to 3% company match and profit-sharing contributions </li><li>Comprehensive paid time off, including vacation and sick leave </li><li>Supportive work environment with professional growth opportunities </li></ul> <p>How to Apply:</p> <p>To apply, fill out the form on this page and submit your resume and letter of interest.</p>
