This position is critical to the delivery of quality psychosocial care and management of patients receiving cancer care at our regional sites.</p> <p>A caregiver in this position works days, 8:00AM to 4:30PM.</p> <p>A caregiver who excels in this role will:</p> <ul> <li> <p>Work collaboratively with healthcare teams to assess and plan interventions to help patients cope with social, emotional, financial and environmental problems.</p> </li><li> <p>Perform psychosocial assessments and provide interventions to assist patients and families. This may include counseling, education, advocacy, referrals, conflict resolution, crisis intervention, problem solving and coordinating family/team meetings.</p> </li><li> <p>Provide and/or document treatment recommendations, professional recommendations, care coordination interventions and treatment plans.</p> </li><li> <p>Engage patients and families in adhering to the treatment plan by reducing or eliminating barriers.</p> </li><li> <p>Maintain a network of resources for consultation and connect patients with community resources.</p> </li><li> <p>Supervise social work interns and assist with training new caregivers.</p> </li><li> <p>Educate other caregivers about the impact of social problems on health outcomes and the effectiveness of social interventions in improving outcomes.</p> </li></ul> <p>Minimum qualifications for the ideal future caregiver include:</p> <ul> <li> <p>Master's degree in social work from a CSWE accredited program</p> </li><li> <p>State Licensed Social Worker (LSW)</p> </li><li> <p>Two years of experience in a hospital or healthcare setting</p> </li><li> <p>Licensed Independent Social Worker (LISW) within five years of hire</p> </li></ul> <p>Our caregivers continue to create the best outcomes for our patients across each of our facilities.
