Working with primary care providers, population health coordinators, high-risk nursing programs, and community health workers, the pharmacists positively impact patients through virtual and in-person appointments. Clinical Pharmacists serve as a departmental resources and liaisons to other departments, hospital personnel, or external groups. They proficiently manage and facilitate the appropriate use of medications according to professional and regulatory standards. They also conduct clinical research and practice advancement projects as well as patient care quality and regulatory compliance initiatives designed to improve medication-use processes or pharmacy practice. Clinical Pharmacists provide medication and practice-related education/training and actively serve as preceptors for pharmacy residents and Doctor of Pharmacy students.</p> <p>Job Summary</p> <p>As a member of the primary care team, the Ambulatory Care Clinical Pharmacist will be responsible for the provision of safe and effective patient centered pharmacotherapy in the primary care space. Clinicians are aligned with targeted interdisciplinary programs and specialty services to manage chronic disease states through Collaborative Drug Therapy Management agreements (CDTM).</p> <p>The following are essential job functions for the Ambulatory Primary Care Clinical Pharmacist:</p> <ul> <li>Leads assigned patient service lines, clinical areas, and therapeutic </li></ul> <p>programs</p> <ul> <li>Delivers direct patient-care and clinical practice, including decentralized </li></ul> <p>and service-based programs</p> <ul> <li>Provide services to ensure optimal transitions of care, including collecting, </li></ul> <p>reconciling, and correcting patient medication discrepancies</p> <ul> <li>Work collaboratively with the patient's provider to establish patient centered </li></ul> <p>disease state goals</p> <ul> <li>Create and implement patient centered therapy plans as outlined by current </li></ul> <p>literature and scope specific agreements, policies, and guidelines</p> <ul> <li>Implements, evaluates, and redesigns pharmaceutical care plans based on </li></ul> <p>patient progress, evolving goals for medication usage, adverse drug</p> <p>reactions, and adherence. This includes adjustment of medications,</p> <p>ordering labs for monitoring, and titration of medications as outlined in the</p> <p>policies and procedures</p> <ul> <li>Accurately orders medications, vital signs, or pertinent laboratory tests for </li></ul> <p>patients</p> <ul> <li>Provides written documentation in the patient's medical record for all </li></ul> <p>patient encounters</p> <ul> <li>Educates patients regarding safe and effective use of medications, </li></ul> <p>possible medication side effects, and the role of prescribed medications in</p> <p>the overall therapeutic care plan</p> <ul> <li>Facilitates experiential education and practice advancement </li><li>Maintains proficiency in the four preceptor and actively emulates </li></ul> <p>education/mentorship skills</p> <ul> <li>Serves as a preceptor/educator for MGH pharmacy residency (PGY1 and </li></ul> <p>PGY2) and student experiential education programs, including maintaining</p> <p>training and experience in pharmacy practice areas appropriate for</p> <p>teaching assignments</p> <p>Qualifications</p> <p>LICENSES, CERTIFICATIONS, and/or REGISTRATIONS:</p> <p>Current licensure as a pharmacist in the Commonwealth of Massachusetts (or eligible) required Board Certification in area of expertise (BCACP, CDE) is required within 18 months of hire (if not already certified)</p> <p>EDUCATION:</p> <p>Graduate of an ACPE accredited College of Pharmacy required. Pharm.D. or other advanced degree preferred Completes required additional continuing education requirements including completion of at least one hour of pain training every 2 years and additional 5 contact hours of CE in the area of credentialing yearly. Maintains active credentialing status as well as MA controlled substance license and completes all required documentation in a timely manner.</p> <p>EXPERIENCE:</p> <p>Completion of ASHP accredited PGY1 Pharmacy Residency required, and PGY-2 Ambulatory Care residency preferred, or equivalent experience Clinical pharmacy experience (1-5 years) in ambulatory care settings and/or CDTM experience preferred; required if no PGY2 residency training Exceptional aptitude or knowledge may be considered in lieu of experience.
