Our focus includes Primary Care, Podiatry, and Behavioral Health. Podiatry provides essential care for our patient population--especially for diabetics--and we have providers currently in the following clinic locations:</p> <ul> <li> <p>Illinois: Ashburn, Avalon Park, Bellwood, Bronzeville, Blue Island, Englewood, Chicago (Madison Street), Hermosa</p> </li><li> <p>Indiana: Fort Wayne, Gary, Glendale, Hammond, IRV, Speedway, UH</p> </li><li> <p>Ohio: Glendale, Lee Harvard, Westown</p> </li></ul> <p>There are also plans for expansion throughout the country.</p> <p>We start simple--build meaningful relationships with patients and their families and then layer on resources to help our teams produce excellent care.
