We have served the community for 55 years, and we pride ourselves on the care that we have provided.</p> <p>We are seeking a detail-oriented Nurse to join our team as our MDS Coordinator. This is a salaried (exempt) position with a base Monday to Friday schedule.</p> <p>Compensation and Schedule:</p> <ul> <li>Salary Range: $65,000 - 85,000 (depending on experience) </li><li>Schedule: Salaried </li></ul> <p>Note: The MDS Coordinator participates in the on-call rotation</p> <p>Essential Functions: Oversees scheduling of the MDS assessments to maintain federal and state regulatory compliance. Coordinates all functions of the MDS assessment and care planning process. Participates in and/or coordinates interdisciplinary meetings. Participates in determining resident level of care. Conducts care plan conferences. Participates in tracking resident events such as falls, antibiotic use, psychotropic drug use and significant changes.
