<p>POSITION SUMMARY:</p> <p>Provide individual counseling, group counseling and case management services to individuals with</p> <p>Substance Use Disorders (SUD) so they may achieve sustained remission from SUD, restore and increase</p> <p>healthy social connections, obtain stable income and permanent housing, and reduce their risk of</p> <p>homelessness. As a Level II Case Counselor also conduct diagnostic assessments, review and sign off on</p> <p>Level I Case Counselors notes, and provide them with mentoring and support.</p> <p>ESSENTIAL FUNCTIONS:</p> <ul> <li>Provide individual counseling, outpatient group counseling, intensive outpatient group counseling, </li></ul> <p>and case management to homeless individuals with substance use disorders.</p> <ul> <li>Provide individual counseling and case management to a case load of program participants. </li><li>Facilitate Outpatient Group Counseling and Intensive Outpatient Group Counseling. </li><li>Record all service activities by entering progress notes in the Y-Haven Electronic Medical Record. </li><li>Establish and maintain regular collaborative relationships with social service organizations and </li></ul> <p>programs that residents may utilize for employment, housing, continuing recovery and other</p> <p>needs.</p> <ul> <li>Participate in daily team meetings, quality assurance meetings, in-service trainings and </li></ul> <p>coordination meetings.</p> <ul> <li>Coordinate with Clinical Director on all critical clinical issues. </li><li>Conduct Diagnostic Assessments. </li><li>Review and sign off on Level I Case Counselor Notes. </li><li>Provide mentoring to Level I Case Counselors. </li><li>Other duties as assigned. </li></ul> <p>QUALIFICATIONS:</p> <p>Education:</p> <ul> <li>Bachelor's Degree Required </li></ul> <p>Experience:</p> <ul> <li>Must have at least 2 years' experience in SUD Counseling. </li></ul> <p>Certification(s) & Requirements:</p> <ul> <li>Minimum License: LCDCIII (Licensed Chemical Dependency Counselor III), LSW (Licensed Social </li></ul> <p>Worker), LPC (Licensed Professional Counselor).</p> <ul> <li>CPR and First Aid certification and Safety training required within 30 days of hire. </li><li>Must be free from sanctions or exclusions from the Office of the Inspector General (OIG) and the </li></ul> <p>State of Ohio Department of Medicaid.</p> <ul> <li>Technical Skills and Knowledge: </li><li>Bilingual (Spanish/English) preferred, but not required </li><li>Proven ability to meet minimum productivity requirements. </li><li>Must have successful experience facilitating SUD group counseling. </li><li>Must have extensive knowledge of Substance Use Disorders and other issues affecting the </li><li>Proven ability to meet minimum productivity requirements. </li><li>Must have successful experience facilitating SUD group counseling. </li><li>Must have extensive knowledge of Substance Use Disorders and other issues affecting the </li></ul> <p>homeless population.</p> <ul> <li>Must have basic computer skills, using Microsoft Office applications. </li><li>Should have working knowledge of OHMAS, CARF and VA program compliance standards. </li><li>Demonstrate commitment to the mission, cause of the YMCA, and uphold its values and ethics. </li><li>Ability to work effectively work with people of different backgrounds, abilities, opinions and </li></ul> <p>perceptions</p> <ul> <li>Willingness and ability to be a mandated reporter for any signs or suspicion of child abuse and </li></ul> <p>neglect. All employees are required to follow the child abuse and neglect reporting process:</p> <p>completion of Suspected Child Abuse/Neglect reporting form, calling children protective services</p> <p>(696-Kids) and notifying your immediate supervisor, branch executive, risk management and</p> <p>human resources department (s).</p> <p>PHYSICAL DEMANDS:</p> <ul> <li>Talking. Expressing or exchanging ideas by means of the spoken word. Those activities in which </li></ul> <p>they must convey detailed or important spoken instructions to other workers accurately, loudly, or</p> <p>quickly.</p> <ul> <li>Hearing. Perceiving the nature of sounds at normal speaking levels with or without correction. </li></ul> <p>Ability to receive detailed information through oral communication, and to make the</p> <p>discriminations in sound.A frequent amount of standing, walking, reaching, stooping, kneeling,</p> <p>and bending.</p> <ul> <li>Sedentary work. Exerting up to 10 pounds of force occasionally and/or negligible amount of force </li></ul> <p>frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human</p> <p>body. Sedentary work involves sitting most of the time. Jobs are sedentary if walking and</p> <p>standing are required only occasionally and all other sedentary criteria are met.</p> <ul> <li>The worker is required to have close visual acuity to perform an activity such as: preparing and </li></ul> <p>analyzing data and figures; transcribing; viewing a computer terminal; extensive reading; visual</p> <p>inspection involving small defects, small parts, and/or operation of machines (including</p> <p>inspection); using measurement devices; and/or assembly or fabrication parts at distances close</p> <p>to the eyes.</p> <p>WORK ENVIRONMENT:</p> <ul> <li>The worker is not substantially exposed to adverse environmental conditions (such as in typical </li></ul> <p>office or administrative work.)</p> <p>DISCLAIMER:</p> <p>The above statements describe the general nature and level of work only.
