This position offers the opportunity to make a significant impact in managing and supporting patients with diabetes and other endocrine conditions in a collaborative and patient-centered environment. New graduates are welcome to apply!</p> <p>Key Responsibilities:</p> <ul> <li> <p>Collaborate with physicians in managing patients with diabetes and other endocrine disorders by providing comprehensive clinical care.</p> </li><li> <p>Lead insulin teaching sessions, diabetes education, and pump initiation in partnership with our Certified Diabetes Educator team.</p> </li><li> <p>Address patient phone inquiries, resolve clinical questions, adjust medications according to protocol, and coordinate ongoing care.</p> </li><li> <p>Work collaboratively with physicians, dietitians, and educators to ensure continuous care and empower patients to manage their condition effectively.</p> </li><li> <p>Perform thorough assessments, including patient history, physical exams, and review of lab results, to diagnose and manage diabetic and endocrine conditions.</p> </li><li> <p>Track patient responses to treatments and provide necessary follow-up care to optimize health outcomes.</p> </li></ul> <p>This is an exciting opportunity to be part of a dynamic, interdisciplinary team in an academic medical center setting, where you will contribute to advancing care for individuals with diabetes and other endocrine disorders.
