<p>Castle Senior Living Hospice is looking for an experienced and passionate MSW - Social Worker. This individual will encompass all that Hospice is about by providing skilled and compassionate psychosocial support to patients and their families.</p> <p>This individual will be Full-Time position working business hours with on-call requirements.</p> <p>Primary Duties:</p> <ul> <li>Interview and complete initial assessment for hospice clients </li><li>Develops assessments, education and ongoing evaluations of patients' needs </li><li>Acts as liaison and resource, providing training to employees, families and vendors </li><li>Evaluates and monitors family relationships, assisting with cultural and spiritual approaches in care and illness through end of life </li><li>Oversight of BSW employee/visits as applicable </li></ul> <p>If you meet these qualifications, we want to meet you!</p> <ul> <li>Master's Degree from accredited school of Social Work or related field; Valid license/registration of permit to practice in state(s) of agency operation </li><li>Medicare/Medicaid/Insurance specialty preferred </li><li>CPR certification preferred </li></ul> <p>Required Certifications and Licensures:</p> <ul> <li>Licensed to practice as a clinical social worker in the state of Wisconsin </li><li>Must be a licensed driver who can travel to all business locations </li></ul> <p>We are an equal opportunity employer and does not discriminate against otherwise qualified applicants based on race, color, creed, religion, national origin, sex, pregnancy, marital status, status with regard to public assistance, disability, sexual orientation, age, arrest or conviction record or handicap, genetic or military status.
