<p>As a Clinical Care Manager, you will lead a team of dedicated clinicians in delivering exceptional client care. You will manage a case load of clients, partner with an interdisciplinary team, train and onboard new hires, and manage client referrals. This is a great opportunity for a collaborative, solution-focused RN leader to make a meaningful impact.</p> <p>In this role, you will:</p> <ul> <li>Supervise and support direct care staff to ensure compliance with policies and regulations. </li><li>Manage a caseload of clients, develop personalized treatment plans, and coordinate care with external providers. </li><li>Conduct comprehensive assessments and supervisory visits, documenting care plans in line with agency standards. </li><li>Lead orientation, training, and competency verification for new hires. </li><li>Act as a liaison among staff, community, and healthcare providers to ensure seamless communication. </li><li>Provide in field supervision and real-time guidance to clinicians. </li><li>Participate in quality assurance, performance improvement, and staff development initiatives. </li><li>Support on-call operations, ensuring timely follow-up and accurate documentation. </li></ul> <p>Qualifications</p> <ul> <li>1+ years healthcare experience </li><li>Registered Nurse (RN) required </li><li>Home health or pediatric experience preferred </li></ul> <p>Location and Travel Requirements</p> <p>This is an on-site position at our Springfield, PA office.
