And above all, it means exceptional care, simply delivered - and we couldn't do it without you.</p> <ul> <li>Where you fit in: </li><li>Assist in providing direct patient care alongside a pharmacist in a doctor's office. </li><li>As an intern you will have the opportunity to participate in nicotine cessation virtual visits, medication therapy management (MTM) calls, patient scheduling, managing population health initiatives, and quality improvement and research activities as needs arise. </li><li>Communicate effectively with pharmacists and other health care professionals and works to improve the medication use process. </li><li>Other responsibilities are carried out as required for the effective, safe, or efficient functioning and operation of the ambulatory pharmacy team. </li></ul> <p>Your Exceptional skills and Qualifications:</p> <ul> <li>In this role, the pharmacy intern is expected to be onsite with the potential for virtual opportunities as they arise. </li><li>Provide medication therapy management services under pharmacist supervision </li><li>Conduct patient phone calls to complete medication reviews or schedule for future visit </li><li>Review patients' medication lists for potential drug-related problems </li><li>Communicate drug related problems with supervising pharmacist and clinic staff, when necessary </li><li>Participate in performance improvement and population health initiatives </li><li>Work with patients and Network resources to overcome medication adherence issues </li></ul> <p>Apply today for immediate consideration.</p> <p>Why Community?</p> <p>At Community Health Network, we build teams that deliver exceptional care through empathy, communication and collaboration.
